
Grant Application
The purpose of this grant is to provide members of Ups and Down with funding assistance when these funds are not available through government or community resources. It is our hope that this grant will assist with the enriching the lives of individuals with Down syndrome as well as those who care for them.
With the generous donations, it is possible to directly benefit individuals with Down syndrome with whatever each individual or family deems important to them by making financial assistance available.
We hope this financial assistance will play a role in allowing individuals with Down syndrome the ability to reach greater independence in the community.
Criteria
1. Must directly benefit the individual with Down syndrome; in the case of a minor, the parent or guardian can apply on their behalf.
2. The individual applying must be a member of Ups and Downs, in good standing, for one year. All members in good standing are eligible to apply (the case of a board member applying for the grant, that member will abstain from voting on the eligibility to receive the grant)
3. Preference will be given to those members who have actively participated on a volunteer basis in some capacity for Ups and Downs as well as those who attend the current Annual General Meeting.
4. The request must not fall under the mandate of other funding sources, ie.: FSCD, Alberta Aids to Daily Living.
5. Grants will be awarded to a maximum of $500.00 per individual per calendar year.
Procedures
1. Fill in grant application and mail it to: Ups and Downs, Grant Application, 1940 – 6th Avenue NW, PO Box 61180, Kensington RPO, Calgary AB T2N 0W3.
2. We require a letter indicating how the grant will help to enrich the life of individual with Down syndrome.
A. For Pre-approvals (for those individuals wishing to pre-approve your request prior to spending the funds). Please submit the grant application & letter as requested in steps 1 & 2. We will then send you a letter indicating if your request has been approved or not. Once your request is approved please submit a copy of the pre-approval letter along with a copy of your original receipts for reimbursement to: Ups and Downs, Grant Application, 1940 – 6th Avenue NW, PO Box 61180, Kensington RPO, Calgary, AB, T2N 0W3.
B. For approvals (for those individuals who have already spent the funds and are now seeking to find if they may receive reimbursement). Please submit the grant application & letter as requested in steps 1 & 2, along with your original receipts. We will then send you a letter indicating if your request is approved or not, if you request is approved you will also receive payment included with the letter at that time.
The Executive Board of Ups and Downs will govern this grant. The Ups and Downs board will review the application and has the final decision to which applicants the grant will be awarded. Each year the Executive Board will allocate funding for the grant in the budget presented at the Annual General Meeting.
